Is Dental Insurance Worth the Cost?

If you're one of the millions of Americans with no dental insurance, is a dental policy for your teeth worth the cost? The answer really depends on what dental work you need, what kind of dental plan you have, whether or not your company is paying the premiums, and the kind of dentist office you frequent.

“Most dental insurance policies emphasize prevention and diagnostics, typically covering two annual exams and cleanings, plus X-rays and, for children and older adults, fluoride treatments”, says Evelyn Ireland, executive director of the National Association of Dental Plans (NADP). Fewer than 6 out of every 10 Americans had dental benefits in 2010, according to a report released in late 2011. Dental policies vary widely, and choosing the right one can be tougher than remembering to brush twice a day.

What's covered, what's not

People with dental insurance commonly have what's described as "100-80-50" coverage, meaning it pays up to 100 percent of the national-average for routine preventive and diagnostic dental care, such as checkups and dental cleanings; covers 80% of the national-average for fillings, root canals and other basic dental procedures; and 50% of the national-average for crowns, bridges and major procedures, Ireland says.

The vast majority of coverage is provided through employee and group policies, plans that charged annual premiums of between $234 and $432 per person in 2011, according to NADP estimates.

One can easily imagine that the same work in a rural community in the Midwest has a much lower fee than a cosmopolitan practice in the Northeast. For many of the dentists in the Boston Area, dental insurance falls short of meeting the needs of their insured clients.” He goes on to state, “many of my patients have elected to drop their coverage and pay out of pocket for their treatment. Most insurance companies have not increased their yearly-maximums in the last 25 years while the cost of many dental procedures has quadrupled.” Cosmetic dentistry is rarely if ever covered by insurance, according to Ireland and McLean.

Dental insurance premiums can be more expensive than simply paying out of your own pocket for routine checkups and cleanings. The cost for you to buy an individual policy averages about $360 a year, McLean says. Meanwhile, paying out of your own pocket for two exams and cleanings and a set of X-rays in 2011 would have cost about $370, on average, according to the American Dental Association.

For care that goes beyond the routine dental care, most plans cap coverage at $1,500 a year, although higher annual limits can be arranged by paying a higher premium. Unlike health insurance, dental plans don't bar coverage for pre-existing conditions, though some policies may restrict coverage for people with missing teeth.

Tooth Colored Fillings or Silver Fillings

Silver amalgam, composite tooth-colored resin and porcelain CAD/CAM are the most common materials used for tooth fillings. There are dental situations where one material is preferable over the other. But what is the difference between these materials?

The ADA official position on silver amalgam is that it is a "safe, durable and cost-effective” material that dentists can use in tooth restorations. Silver amalgam is a more-durable tooth filling material for back teeth which are subjected to a lot of biting pressure. Also, amalgam fillings costs considerably less than the same, tooth colored resin filling or ceramic filling. However, silver fillings contain mercury, are unsightly and lead to vertical cracking of the teeth over time.

Tooth colored resin fillings result in a natural-looking smile. The color can be matched to your natural teeth, so the restoration is undetectable. And because the resin compound actually bonds to your tooth, the seal is tighter. However, the bond between the plastic and tooth deteriorates rapidly due to disproportionate expansion properties and does not hold up well under the ‘load & pressure’ during biting/chewing. The durability of resin tooth-filling material has not been time-tested, but improvements are making resins stronger.

Ceramics, especially those done with the state of the art CAD/CAM procedure, can be the best of options. They are beautiful, durable and long lasting. However, since technology is involved and often a laboratory is utilized, they are more-costly than the resin or amalgam.

The ADA is aware of a recent study that associates yearly or more frequent dental X-rays to an increased risk of developing meningioma, the most commonly diagnosed brain tumor. Suffering from Tooth Pain? I did...and it wasn’t fun!

Have you ever had tooth pain? Sometimes a toothache is caused by the metal fillings that have been in your mouth for years. The metal in your tooth causes your teeth to crack. As you bite down, the crack opens and spreads the tooth-walls, causing the pain in your teeth, mouth and jaw. It can be debilitating! Often times, it is difficult to diagnose the tooth that is causing the pain because the nerves in your teeth and mouth are so intertwined that your brain can not identify which tooth the pain is coming from.

I’ve been suffering from tooth pain for over a year. I went to my general dentist last week because the pain was so severe. The tooth causing the pain could not be identified so he recommended I see an endodontist (root canal specialist). There, they “identified” a “suspicious” tooth and scheduled me for an emergency root canal.

I did not arrange the root canal procedure because I was not comfortable with the diagnosis. A tooth was identified as the culprit that neither myself nor my general dentist thought was correct.

I’ve heard many people speak with high regard about Dr. Ryne Johnson. I called his office and discussed my lingering discomfort with him. Dr. Johnson said “come see me right now, I’ll figure it out”. I was a bit surprised by his stated-confidence in his own abilities. No one had been able to help me thus far, so I arranged a visit for the next day.

I traveled to his new and beautifully appointed office space in Newton. With ease, he found the tooth that was problematic and was even able to show me the offending tooth via a digital camera that projected the image on a chair-side, computer screen. The tooth had an obvious crack from the top of the tooth down to the gum line. Dr. Johnson said diagnosis is called “cracked tooth syndrome”. Because I could physically see the problem, I was immediately confident in his diagnosis and accepted his recommendation regarding treatment.

Dr. Johnson began his procedure, shaped the tooth and then showed me the crack again; which went all the way through the tooth, from one side to another. If I had waited, the tooth would have broken in half and I would be facing a dental implant procedure at significantly more cost.

Dr. Johnson, finished his visit, put a temporary cap on my tooth and now, the bite feels perfect. As I left, an hour after my arrival, he informed me that he would call me in 2 weeks for an update. If I’m comfortable, he’ll schedule me for the ceramic, CAD/CAM created dentistry.

I never knew of a “prosthodontist”. Nor did I realize that a prosthodontist has additional training and expertise than a general dentist. After today’s experience, I can tell you, first-hand, that there is a HUGE difference. My experience with Dr. Johnson, couldn’t have been better and I now understand that I should have seen a prosthodontist from the start… and I escaped a root canal!

A study done by the State of the Cosmetic Dentistry Industry Survey noted that teeth whitening procedures for patients under the age of 20 have increased 12% over the last 5 years.

Q. What causes yellow teeth in kids?

A. The most common reason young people have yellow teeth, besides genetic factors, is the lack of good brushing and flossing habits. Another cause may be the antibiotics that many teens take for acne. Also if a teen drinks a lot of dark colored soda or pigmented foods such as tomato sauce, that can impact tooth color as well. Some Toothpaste can be Hazardous to Your Health

According to Dr. Ryne Johnson of Newton Prosthodontics, the FDA is warning that some international companies are putting antifreeze in ‘knock-off’ oral hygiene products. A list of the brands of toothpaste from China that contain DEG and are included in the import alert have been identified by the FDA and can be found below. The products typically are sold at low-cost, “bargain” retail outlets.

The Toothpaste is contaminated with a bacteria that could pose “a significant risk to health,” according the FDA. In a separate announcement, the FDA has also said it has asked border guards to block imports of Chinese toothpaste because the department has found 21 samples contaminated with a poison.

The counterfeit Colgate toothpaste has been removed from the shelves of the Dollar discount chain, where it was being sold, the department said. The Chinese toothpaste is sold in Chinese markets and dollar stores across the country. The toothpaste is labeled mainly in Chinese, and there have been no reports of adverse reactions.

The 21 samples contain up to 13.7% diethylene glycol (DEG), used in antifreeze and as a solvent. Genuine Colgate toothpaste is labeled in English. Consumers should not use toothpaste with the incorrect markings.

The risks from both the Chinese toothpaste and the fake Colgate toothpaste are higher for children and vulnerable populations, such as patients with severe underlying diseases or with compromised immune systems. “While toothpaste is not intended to be swallowed, it is often swallowed by young children,” Swallowing the toothpaste could cause nausea, abdominal pain, dizziness, urinary problems, kidney failure, breathing problems, lethargy, convulsions, coma and even death, the department’s release said.

Osteoporosis Medications and Your Dental Health

As we grow older, our bones begin to lose their density and strength, especially after we reach our 50's. Many factors affect bone density, including diet, physical activity, family history, hormones, lifestyle and certain conditions and medications.

Osteoporosis affects about 10 million Americans, 8 million of whom are women. This disease affects more women than cancer, heart disease and stroke combined. Another 34 million Americans have osteopenia, or low bone density and are at risk of developing osteoporosis.

Bad Breath can be a Sign of Significant Issues

Halitosis is the technical name for bad breath and is the third-most-frequent reason for going to the dentist, behind tooth decay and periodontal disease.

Throughout the day, the intensity of bad breath changes as a result of food eaten, obesity, digestive disorders, one’s gum and bone health, smoking, and alcohol consumption. Bad breath may come and go or it may be persistent. Some bad breath disappears after eating, brushing, flossing, or rinsing with mouthwash.

Most people who visit the dentist as a result of bad breath do so because it is persistent or chronic. According to Dr. Ryne Johson of Newton Prosthodontics, halitosis can be a sign of a more serious condition.

Although bacteria normally collects on your tongue, gum and teeth and causes a bad odor, people with healthy gums can remove this bacteria by brushing and scraping their tongue while brushing their teeth. Over the past several years, significant research has been completed that evaluates the relationship between dental health and overall health. According to Dr. Ryne Johnson of Newton Prosthodontics, many people are unaware that gum disease can lead to cardiac problems, stroke or even prostate cancer.

Heart Disease

Several theories exist to explain the link between periodontal disease and heart disease. One theory is that oral bacteria can affect the heart when they enter the blood stream, attaching to fatty plaques in the coronary arteries (heart blood vessels) and contributing to clot formation. Coronary artery disease is characterized by a thickening of the walls of the coronary arteries due to the buildup of fatty proteins. Blood clots can obstruct normal blood flow, restricting the amount of nutrients and oxygen required for the heart to function properly. This may lead to heart attacks.

Another possibility is that the inflammation caused by periodontal disease increases plaque build up, which may contribute to swelling of the arteries.

Researchers have found that people with periodontal disease are almost twice as likely to suffer from coronary artery disease as those without periodontal disease.

Periodontal disease can also exacerbate existing heart conditions. Patients at risk for infective endocarditis may require antibiotics prior to dental procedures. Stroke

Additional studies have pointed to a relationship between periodontal disease and stroke. In one study that looked at the causal relationship of oral infection as a risk factor for stroke, people diagnosed with acute cerebrovascular ischemia were found more likely to have an oral infection when compared to those in the control group.

Prostate Cancer

Researchers from Case Western Reserve University School of Dental Medicine and University Hospitals Case Medical Center report initial results from a small sample that inflammation from gum disease and prostate problems just might be linked. They discuss their new evidence in the Journal of Periodontology, the official journal of the American Academy of Periodontology.

So a while back I was at a Fisher House event and I met John is a big time investment banker, executive here in Boston.
